Dorothy L. (Mazer) Forman, 94, of Meyersdale, died June 12, 2017 at MeadowView Nursing Center, Berlin. Born October 31, 1922 in Larimer Twp., the daughter of William Howard and Elizabeth (Suder) Mazer.

Preceded in death by her parents; husband, C.E. “Pat” Forman; son, John Howard Forman; granddaughter: Sandy (King) Johnson; brothers: Ralph, Merle, Kenneth, Howard, and Homer; sisters: Margaret Mazer, Mary Long, Ruth Kinsinger, Edith Barmoy, and Alma Meyers.

She is survived by her children: Linda K. Forman, Peggy J. (Jack) Beitzel, David K. (Kathy) Forman, all of Meyersdale; and Jay H. (Robin) Forman, Vienna, VA; grandchildren and great grandchildren: James Johnson and children, Catherine and Jackson, Garrett; Patrick Forman, Meyersdale; Joseph (Jess) Forman and daughter Emma, Irwin; Brittany Beitzel and fiancé Hunter Booth and children: Ariana and Chase Booth, Rockwood; sister: Zona (Homer) Boger, Meyersdale; sister-in-law: LuAnn (Forman) Jackson, Laramie, WY; several dozen nieces and nephews. Dorothy was a homemaker and enjoyed gardening, crocheting, playing cards, and reading. Member of Zion Lutheran Church, Meyersdale and Trinity Chapter 138 Eastern Star.

Friends will be received from 4 to 8 p.m. Wednesday at Price Funeral Service, Meyersdale, where service will be conducted at 3 p.m. Thursday, with Pastor Pete Kaltenbaugh officiating. Interment Mt. Carmel Cemetery at White Oak. Memorial contributions may be given to the Children’s Institute, 1405 Shady Ave., Pittsburgh, PA 15217 for the Prader Willi Wing.
